Services we provide
Rapid Response is a community support, rehabilitation and subacute care service. It is staffed by registered nurses, physiotherapists, occupational therapists and healthcare assistants. The service can access input from healthcare practitioners across many specialties.
We can support you through:
- your recovery from injury or surgery
- exacerbations (worsening) of chronic illnesses like heart failure or lung disease (for example, chronic obstructive pulmonary disease or COPD).
Our focus is on early supported discharge and on avoiding the need for you to be admitted to hospital.
We create a care plan that features a nursing and allied health component. The care plan is fulfilled by registered health providers and healthcare assistants. General practitioners and nurse practitioners provide clinical support.
Referral information
Referrals to the Rapid Response community care service can be made through an assessment by the Integrated Community Assessment Treatment Team (iCATT).
